A practice built from lived experience

HH Monn Care was founded by Hannah Monn, a former ESG analyst with a background in economics and systems thinking. The practice was not born from a career pivot. It was born from a week in December.

When her grandfather was discharged from a rehabilitation facility, she flew to Georgia and stayed a week to manage the transition. What she found wasn't a family in crisis — it was something more common and more dangerous: a family that didn't know how much help was actually needed. No coordination infrastructure. No clear communication. No single person with a full picture.

She handled the discharge, scheduled follow-up appointments, arranged PT and nursing visits, and identified a home care agency. Then she turned to the medications. A review across three pharmacies revealed the list didn't match what had been prescribed. Pill organizers contained errors — doses skipped, others doubled. She audited everything, contacted physicians and pharmacists, reconciled the discrepancies, and consolidated all prescriptions to a single pharmacy with pre-packaged monthly pill packs.

 

On subsequent visits, she navigated dual dementia and Alzheimer's diagnoses, advocated for 24/7 care, managed caregiver reliability issues, facilitated powers of attorney and HIPAA releases, filed DNRs, wrote detailed care plans for staff, reorganized the home for safety — and fixed the family communication. She built a shared coordination system so that everyone, regardless of where they lived, always knew exactly what was happening.

That experience is HH Monn Care's most important credential. Every service offered has been performed, tested, and refined through it.
